Now You Can Switch Online & Get a Free Choose Life plate frame (White or Silver)

There are three sets of instructions on this page.

The first set of instructions tell you how to switch at  your county license plate office.

Email to Russ Amerling at russ@choose-life.org or call him toll free 877 454-1203 for any questions you may have about the Choose Life license plate. To switch at your county license plate office, take your registration and proof of insurance to the office nearest you. You will pay $40.10, which includes the one time $18.10 switching fee and the annual fee for the Choose Life license plate of $22. If it happens to be within 3 calendar months of your renewal date (i.e. birth date of the person whose name is first on your vehicle registration) you can renew and switch. You would then pay the renewal fee (found on last year's registration or the new renewal notice if you have received it) plus $32.00.  $20 of the $22 annual fee is a tax deductible donation to the Pregnancy Care Centers, Maternity Homes and certain non-profit adoption agencies within your county. After you switch, email russ@choose-life.org your new Choose Life plate number along with your daytime phone number and address and you will receive a free white or silver plastic Choose Life license plate frame (see Product Store on our homepage).

The second set of instructions tell you how to switch online for a small online service fee.

NOTE! Read all these instructions before you click on the Point and Pay link below. You can search for your registration with a PIN number (found on the renewal notice you receive from the DMV yearly) or by your birthday and license plate number. Click on #1 below. If neither of these methods work for you click on # 2 link below and manually enter in your vehicle data. HOWEVER, BE SURE TO TELL THEM YOU WANT TO SWITCH TO THE CHOOSE LIFE LICENSE PLATE, IN THE COMMENTS SECTION AT THE BOTTOM OF THE FORM. IF YOU DO NOT KNOW THE AMOUNT, YOU CAN PUT $40.10 IN THE AMOUNT BLOCK AND ADD THIS STATEMENT IN THE COMMENTS SECTION: "I GIVE MY PERMISSION TO POLK COUNTY TAG OFFICE TO CORRECT THIS AMOUNT TO COVER THE COST OF SWITCHING TO THE CHOOSE LIFE LICENSE PLATE. PLEASE EMAIL ME WITH THE AMOUNT YOU HAVE CHARGE TO MY CREDIT CARD."

Then follow the online prompts. You will be able to switch to the Choose Life license plate online and pay with your credit card. You will be charged a small fee of approximately $2 online service charge, plus the same fees outlined in the instructions above. When your file comes up, click on the Florida Plate under the column that says "Click to Change Tag Type" and then click on the column "Organizational" to see the Choose Life plate. Click on that and it will take you back to complete the order process. Once you complete the transaction, if you cut and paste the receipt they give you into an email (and add your mailing address and daytime phone number) to russ@choose-life.org, Choose Life, Inc. will send you a free white or silver plastic Choose Life license plate frame (see Product Store on our homepage). The third set of instructions are for qualified agencies in Florida who are trying to access the Choose Life funds from their County Commission.

Choose Life, Inc. is willing to help potentially qualified agencies within the state of Florida to help them claim a portion of the funds raised by the sale of the Choose Life license plates in Florida. This money is given to the County Commissions by the Department of Motor Vehicles annually in July or August. Current sales are shown on our "Newsletter" page, so you will know how much is being raised in your county.

Choose Life, Inc. does not make any determination as to who is a qualified agency. The statute passed by the Florida legislature states what is a qualified agency and how they may spend the funds. (see Bill).

General Recommendations for Requesting Funds from Choose Life License Plate Sales

Most important! If there is a "Lead Agency" already established in your county, contact them and join the coalition. If not, then try to get the other qualified agency directors together and elect one. The Lead Agency then works with the county and that greatly simplifies the paperwork process.

Contact Russ Amerling for a list of these agencies in your county. Make every attempt to avoid disagreements concerning the distribution. County Commissions have the responsibility to distribute the funds to the qualified agencies within the county. Therefore, if an agreement among these agencies is reached, they usually will have no problem in making the decision of what to do with the funds. It is imperative that all qualified agencies work together to arrive at the best way to spend the limited resources. It is our belief that agencies should be sensitive to other qualified agencies in their area. We recommend negotiation amongst qualified agencies prior to any requests for funds being made to the County Commission. Choose Life, Inc. is available to attend meetings to answer questions and furnish you with forms and procedures already adopted in other counties..
